The Decade You Pay For At The End

A doctor told me my numbers were fine. I was fifty, and fine meant every marker sat inside a range built from a population that is, on average, unwell. Insulin resistance runs for five to ten years before fasting glucose moves at all. In older men a hip fracture carries a 30 percent one-year mortality, which is the number nobody puts on the poster. The marginal decade is either paid for in advance or financed at the end.

Diagnosis Is Not The Start

A cardiologist told me my numbers were fine. I was fifty, I had just finished restructuring a EUR 75 million industrial group, and fine meant every marker sat inside a range built from a population that is, on average, unwell. That is Medicine 2.0. It waits for the disease to cross a line, then treats the line. The line is called the disease threshold, and by the time you cross it the plaque has been accumulating since childhood. Peter Attia's Medicine 3.0 runs the other way. It intervenes a decade or more before the diagnosis and it accepts weaker evidence to do it: mechanistic data, Mendelian randomisation, epidemiology, not only randomised trials. The target is not lifespan. It is the marginal decade, the last ten years, and the entire bet is that you spend them walking instead of being carried. The pattern is identical to every collapse I have ever traded through. The 2013 Cyprus bail-in did not begin on the morning the banks closed. It began years earlier, in a balance sheet nobody wanted to read.

ApoB, Not LDL

Ask for the wrong lipid panel and you will be reassured by it. LDL-C is a calculation, and it is discordant with real risk often enough to matter. ApoB is a count: one molecule on every atherogenic particle, LDL, VLDL, IDL and Lp(a) alike. It is the causal variable, not a proxy for one. Attia's personal target is below 40 mg/dL, which is paediatric territory. For patients he will accept 60 to 80. Below 80 mg/dL is where the conversation starts, not where it ends. Then there is Lp(a). Roughly 90 percent genetic, decided by the age of two, elevated in about one person in five, and almost unmoved by diet or training. It is also pro-thrombotic: the apo(a) arm resembles plasminogen and blocks the system that dissolves clots. One test, once in a lifetime, above 50 mg/dL or 125 nmol/L, and it rewrites the lipid target for everything else. There is aneurysm and cancer on both sides of my family. Sixteen years of rural Nigeria, post-conflict Serbia, Kazan and Fukushima taught me that risk is priced by whoever bothered to read the file. I had never read mine.

Fasting Glucose Is The Last Thing To Move

Fasting glucose is the last thing to break, not the first. Insulin resistance can run for five to ten years before the fasting number leaves the normal range, and by then the damage is a decade old. The test that catches it costs one morning. Seventy-five grams of glucose, blood drawn at 0, 30, 60, 90 and 120 minutes, measuring insulin as well as glucose. Dr Joseph Kraft used exactly this to describe five insulin patterns. Pattern I is a clean peak at 30 to 60 minutes and a return to baseline inside two hours. Patterns II to IV are the quiet slide, and they show up years before anyone says the word diabetes. Attia's bars sit tighter than the lab's. Two-hour glucose under 120 mg/dL, where the guideline says 140. Fasting insulin under 6 µIU/mL. HbA1c under 5.4 percent. Peak glucose under 160 and back down within the hour. Every one of those numbers is a leading indicator, and not one of them hurts to obtain.

Train For The Fall, Not The Heart Attack

Here is the sentence that reorganised my training week. In older men, a hip fracture carries roughly a 30 percent one-year mortality. Not a heart attack. A fall. Attia's centenarian decathlon asks a different question. Name ten physical things you intend to do at 100, then train for them now, with margin. Getting up off the floor without using your hands. Carrying two 5 kilogram bags up four flights of stairs. A 30-second dead hang. Standing on one leg, eyes closed, for half a minute. VO2 max is the strongest single predictor of all-cause mortality in his framework. Each 1 MET of improvement, about 3.5 mL/kg/min, is associated with a 12 to 15 percent reduction in mortality risk. Move from the bottom quartile to above average and that risk falls by more than half. He wants his patients in the top 2.5 percent for their age. I ran the arithmetic on my own frame. At 89 kilograms, a VO2 max of 45 needs an absolute 4 litres of oxygen a minute. That is the range of an average 25-year-old. It is not vanity. It is the difference between the last decade being lived and the last decade being managed.

Fasting glucose is the last thing to break, not the first.

The Stress Belly Is A Balance Sheet

Chronic cortisol is not an emotion. It is insulin resistance with a timeline. It drives visceral fat first, the fat you cannot see, which is why a lean frame can still be metabolically broken. Thin outside, fat inside. Attia's own history is worth more than his protocols. He used achievement as a substitute for self-worth, and it worked, right up to the point it stopped working. Then he checked into a residential trauma programme in Nashville and began weekly therapy. His line: you cannot outrun your demons, you have to turn around and face them. The epidemiology agrees with him. Social isolation carries a mortality risk roughly equivalent to smoking 15 cigarettes a day. The Harvard Grant Study spent decades finding that the quality of a person's relationships predicts late-life health better than cholesterol does. In 2008 I signed a EUR 20 million personal guarantee and the company folded on Christmas Day. For years I filed that under business. It was a cortisol problem with an accounting department, and it is still on the balance sheet.

Buy The Position Before The Symptom

The probability that a serious diagnosis arrives with no warning at all is small. The probability that it arrives after ten or fifteen years of unread warnings is close to one, and most people are pricing it at zero. That is the whole case. Not fear. Pricing. This is not a protocol and I am not handing one out. It is a position I have taken, and the logic is the same logic I have used building companies across twelve countries and through one Frankfurt listing. The tests get ordered before the symptom. The training gets done before the fall. The emotional work gets done before it lands on somebody else. The marginal decade is paid for in advance or financed at the end, and the interest rate at the end is the highest one you will ever see. Somebody asked me what I do. I said I restructure things that are already broken. Then I went home and read my own numbers.
